This is a great opportunity to provide meaningful, community-based care while working with a high level of clinical autonomy and independence.You'll provide a combination of clinical nursing, care coordination, hospital discharge support, home visits, assessments, and community-based services, while collaborating with an interdisciplinary team.Role Highlights:• Conduct nursing assessments and screen individuals for program admission• Coordinate hospital discharges and support transitions back to the community• Conduct home/residential visits and provide ongoing clinical support• Oversee medications, health monitoring, chronic disease management, wound care, and other nursing needs• Coordinate care with hospitals, providers, families, and interdisciplinary teams• Support individuals with intellectual/developmental disabilities (IDD) and severe mental illness (SMI)• Travel independently throughout the five boroughs, primarily via public transportation• Maintain accurate documentation and compliance with OPWDD/OMH/DOH requirementsRequirements:• Current, unrestricted NY RN license• 2+ years of nursing experience• IDD, behavioral health, community health, or crisis experience preferred• Strong clinical assessment and care coordination skills• Excellent clinical judgment and ability to work independently• Strong communication, organization, and documentation skills• Valid driver's license preferredInterested? xhyhwjd
